Our center is open to children of ages 12 months through five years old based on age of the child on or before September 1st of the current school year.
At the time of enrollment, parents must present the following completed information and fees:
1.) Enrollment and Financial Agreement Form
2.) Emergency Treatment Authorization Form
3.) Discipline Policy Acknowledgment
4.) Discipline and Guidance Policy
5.) Photo Release Form
6.) Master Record Form
7.) Physical with physician signature (by physician who has examined the child within the past 12 months) is due one week prior to enrollment
8.) Registration fee payment
9.) Application fee payment
10.) A Beka Book fee (2yrs.-4yrs.)
If there are any changes of policies, you will be notified by letter or memo on door.
